Players:  You need to complete the following forms and bring them with you to the General Meeting. CEVA/USA Volleyball Forms must be completed and you MUST register online.  This is a secured site and you can read about it when you visit their site. Parents:   Please download the Parent Agreement Form after tryouts.  We will review this form during the All Club Meeting.  If your daughter was placed on a High Performance Team, you will also need to download the High Performance Team Agreement Form.
